I Care Community Connections is seeking compassionate, organized, and resourceful Targeted Case Managers (TCMs) to join our growing team. This position is ideal for professionals who are passionate about helping individuals and families navigate behavioral health services, community resources, and other supports needed to improve stability and overall well-being.
This is a 1099 independent contractor opportunity offering the flexibility to create and manage your own schedule while providing meaningful case management services in the community and through a hybrid service model.
Bilingual candidates are preferred but not required.

About The Position
Targeted Case Managers at I Care Community Connections work directly with clients to identify needs, develop individualized service plans, coordinate services, connect clients with community resources, and monitor progress toward established goals.
This position follows a hybrid service model. Services may be provided in the community, at the I Care Community Connections office, and remotely when permitted and appropriate based on client needs, payer requirements, and agency guidelines.
Contractors have the flexibility to create and manage their own schedules while maintaining responsibility for their assigned caseload, required client contacts, documentation, communication, and continuity of services.

Key Responsibilities
• Complete comprehensive assessments of each client’s needs, strengths, resources, barriers, and support systems.
• Develop and maintain individualized service plans based on identified client needs and goals.
• Provide ongoing case management services in accordance with the client’s service plan.
• Connect clients and families with appropriate community resources and support services.
• Assist clients with accessing behavioral health care, medical care, housing resources, transportation, educational services, employment resources, public benefits, social services, and other community-based supports.
• Coordinate services with behavioral health providers, medical providers, schools, social service agencies, community organizations, and other members of the client’s support system when properly authorized.
• Advocate for clients when barriers interfere with access to needed services and resources.
• Monitor client progress and update service plans as needs and circumstances change.
• Conduct required client contacts and follow-up activities within established timeframes.
• Maintain accurate, timely, and complete case management documentation.
• Complete progress notes, assessments, service plans, service plan reviews, discharge documentation, and other required records within established timeframes.
• Participate in required team meetings, supervision, training, and quality improvement activities.
• Maintain professional and appropriate boundaries with clients and families.
• Follow all agency policies and procedures, ethical standards, confidentiality requirements, and applicable federal and Florida laws and regulations.

Qualifications
- Bachelor’s degree or higher in social work, psychology, counseling, human services, sociology, criminal justice, or another qualifying related field.
- Must hold an applicable Florida Targeted Case Management certification or be eligible to obtain the required certification within three (3) months of hire.
- Bilingual candidates are preferred but not required.
- Strong communication, organization, problem-solving, and time-management skills.
- Ability to work independently while remaining connected to a collaborative team.
- Ability to manage a caseload and maintain timely documentation.
- Knowledge of community resources and social service systems is preferred.
- Experience working in behavioral health, social services, child welfare, community-based services, or case management is preferred.
- Reliable transportation and the ability to travel within the service area as needed.
- Reliable access to technology for documentation, communication, and remote work responsibilities.
- Successful completion of all required background screening and onboarding requirements.

Contractor Classification
This position is offered as a 1099 independent contractor opportunity. Contractors are responsible for managing their own schedules and professional obligations in accordance with the independent contractor agreement, applicable payer requirements, certification standards, and agency policies.
